Care Package Mobilisation (Case Managers)
Getting to know your client
First of all, we’ll undertake an initial needs assessment. The client will meet with their local Clinical Lead or Field Care Manager.
They will focus on the personalisation of care for your client. Together with your client, they will develop a care plan. They’ll establish what training is needed for carers, to make sure they can meet your client’s needs and will build a carer profile to make sure the team has the right skills and personal attributes.
Care Planning
A care plan which is a true personalised guide to care and support is created alongside the client. It is built around their choices and allows the team to provide clear and consistent care.
It will also identify any risks, keeping people safe while respecting independence.
Central Support
Clients are also supported by an experienced Central Support Team.
That team includes a Clinical lead, who oversees quality of care, a Local Field Care Manager who is the line manager of Carers and there for whatever the client needs and a Care Co-Ordinator, who manages rotas.
Recruitment and compliance
Everyone who is recruited to work in a care package has a DBS check, we seek references and they will be trained to make sure they meet the specific needs of the client.
Training and preparation
Carers have bespoke training to work on any care package, tailored to a person’s care plan. There are opportunities to shadow with families or experienced carers and there is regular nurse input to ensure confidence and safety.
Communication and updates
There will be regular updates throughout the mobilisation process to keep you and the client informed. There will be ongoing reviews of the care plan, with weekly meetings or phone calls as needed. We believe communication is critical for success.
