Before we begin working with a person, we ensure that their needs have been fully assessed, so that we can provide the right care for them. This may involve working in partnership with them and their family, their case manager and the commissioner of their care to agree the correct structure to meet goals and needs.
As part of our care planning, we focus on goals and aspirations, what the person would like to achieve with their care and support. Goals will be different for everyone. For some, it may be spending time doing something they enjoy, returning to school or work or being more independent with household tasks. For others, it may be taking a trip either in the UK, or overseas.
Whatever a person’s goals are, we will work closely with them and those around them to help achieve those ambitions – and celebrate them.
Support
Our team work with people to thoroughly assess a person’s needs and create a person-centred support plan. This will highlight the areas where a person needs help to achieve their goals and aspirations. Support will look very different for each person but might include support with:
- Self and personal care
- Keeping safe
- Budgeting and travel
- Social skill development
- Educational and vocational development
- Household tasks
- Cooking, meal planning and shopping
- Positive behaviour development
- Managing medication
