For us, providing outstanding specialist complex care that helps people live as well and as safely at home as possible is more than just the gradings we receive.
Outstanding care, for us, means our nurse-led, outcomes-focused specialist complex care is trusted by clients, families, case managers, nominated deputies and commissioners.
It means we choose our teams carefully to meet people’s needs, to be the best ‘fit’ possible and they’re trained to the very highest standards, by sector-specialists.
Outstanding means we focus on offering the right support to help people achieve their goals, whatever they may be, and celebrating with them when those goals are achieved.
And yes – the specialist care offered by Libertatem Healthcare Group has been graded CQC outstanding twice in a row – but we never rest on our laurels. We’re always aiming for better, to improve our care and to go beyond outstanding.
Libertatem – Going beyond outstanding to empower people to thrive, living life to the fullest with comfort, dignity and independence”.
What is specialist complex care?
We offer nurse-led, highly specialist care for children, young people and adults with complex care needs including:
- Specialist and experienced paediatric complex care
- Adult complex care
- Brain and spinal injury
- Long-term and degenerative neurological conditions
- Complex health and social care
We create care packages – provided by Registered Nurses or skilled and trained Healthcare Assistants – which reflect a person’s individual needs, whether that’s 24-hours-a-day, day or night support, 2-1 complex healthcare support or live-in or specific complex care packages. We can offer bespoke care including hourly, waking nights, sleeping night support and more. It’s all about what the person needs. Our teams also support children in schools and colleges. And we offer personal assistant support too.
We support adults, young people and children who might have both complex health and/or social care needs. They might be recovering from life-changing illnesses, injuries or surgery. Some may need long-term continuing healthcare. We also support people as they approach end of their life and require thoughtful and supportive palliative care.
The aim, always, is to support the person to live their life, their way.
Working together to achieve goals
We focus on safety and quality – and helping people to achieve their goals.
Working closely with people, families, case managers, their nominated deputies and commissioners, we create bespoke care packages which reflect a person’s goals, ambitions and needs. We do this across the UK – supporting clients in England, Wales and Northern Ireland. We’re a national provider but have an established, experienced and highly caring local presence wherever we work.
People can be referred to us by case managers, Integrated Care Boards, local authorities or nominated deputies. We also work alongside people who hold their own personal health budgets.
We work with case managers and solicitors to provide support for clients who have sustained
personal injuries because of an accident or medical negligence as well as ICB/NHS funded clients and those with learning disabilities and mental health challenges.
