Spinal injury complex care
Our home care for children, young people, and adults with spinal injuries focuses on maximising independence and achieving individual goals. Spinal Complex Care packages can take different forms, each tailored to the person’s own needs, including 24-hour, day or night and live-in support.
At the beginning of a care package, our team, including an experienced Nurse Manager, will work with the person to create a support plan. It will include goals they want to achieve and aims to support their social, emotional and physical health needs.
Care is adapted to meet the evolving needs of a client. We work with the client to create a team around them based on experience, expertise and ‘fit’.
Clinical support
Our team can support people’s individual clinical needs including supporting with:
- Receiving nutrition and hydration via a gastrostomy tube
- Maintaining a clear airway via tracheostomy or a ventilator
- Using cough assist or suction devices
- Medications
- Movement, positioning or turning
- Behavioural challenges
- Personal care
- Epilepsy, diabetes or other chronic conditions
Working together
In collaboration with mutli-disciplinary teams, we develop packages of care which help people safely move out of hospital or rehabilitation settings and back home, as soon as is clinically appropriate.
We work closely with clients and their families, fully understanding the trust and adaptation it requires for people to have team members within their home. It is a trust and understanding that we greatly value, and do not take for granted.
We also work closely with case managers, nominated deputies, legal teams – both pre and post settlement – to ensure our care is effective and helping to meet the client’s goals. Our teams work closely and collaboratively with other professionals around the client – such as physiotherapists, speech and language therapists, psychologists and others – to help make sure their rehabilitation process is maximised, cohesive and effective.
